New Hire Forms order form instructions:
To order your customized DOT New Hire Forms, please provide the information requested below. Orders are generally processed within 24 hours of being received, but may take longer depending on the volume of orders in process.
Multiple termimals. If you have more than one terminal that will be using the same forms, please complete a separate order form for each terminal. Each terminal will be listed with a check box on the cover page, allowing a single set of forms to serve multiple locations, all for a single fee. If each location requires its own, individual forms, a one-time fee ($60) for each location may apply. Please use the 'Comments' field to provide additional information.
Updates & revisions. Standard & Best will update and revise your forms at no additional cost to you whenever you notify us of company information that requires updating. As federal rules change that require revisions, you will be notified by email of the required changes and be asked to confirm current company information before updates are made.
